Team Gear Order Form
Please complete the below form. A copy will be sent to yourself, our Equipment Director for ordering and our Treasurer who will ensure payment of your order.
Special Instructions:Once the order is submitted, your family account will be deducted within a 24 to 48 hour period. Please ensure adequate funds are in the account to avoid a hold on your order.
Once an order is placed it will take up to 4 to 6 weeks to receive and will be distribtued through your coach to your child.
All orders are a final sale. Any returns or exchanges are only accepted if there has been an error by the supplier with respect to printing or sizing. Please ensure sizing and printing details are accurate prior to submission. FSSC cannot be responsible for any errors on the team gear with respect to sizing and printing.
Personalized Caps - we will only be able to place your order once we have 12 names and 4 of each (48 caps total). If we do not have enough orders when your request is submitted we will hold your request until numbers have been reached.
